MediLink Community Health Services

MediLink Community Health Services, a division of Mezentco Inc., assists in meeting the needs of a changing health-care environment and promoting the role of pharmacists in the broader community. MediLink provides a high level of pharmacist consulting services to community health centres, family health teams, physicians, employers and benefits consultants.

MediLink pharmacists currently are part of Hamilton family health teams, as well as community health centres in Hamilton and Kitchener. They are part of a provincial strategy to provide comprehensive health care and reduce emergency room wait times and unnecessary hospital admissions as a result of medication-related issues. Studies estimate that five to 10 percent of all hospitalizations and emergency room visits are medication-related and preventable.

Patients interact with MediLink pharmacists as part of wide-ranging wellness programs, such as:

Medication management: A detailed review of a patient's medication regimen, for patients at risk of medication-related issues.

Disease state management: The creation and monitoring of patient care plans for diseases like diabetes, hypertension and asthma, which require the intense use of medications.

Wellness promotion: Community presentations, disease screening programs, and supporting patients' responsibilities for their own health and wellness.

What's New

Marchese Health Care Pharmacist Leslie Chappell has become the company’s first pharmacist to be certified by the Commission for Certification in Geriatric Pharmacy.

The Certified Geriatric Pharmacist designation identifies pharmacists with expertise in drug therapy and pharmaceutical care to the elderly. She joins an elite group of pharmacists from around the world who have distinguished themselves as leaders in geriatric care.

Leslie’s certification will add a greater level of expertise and service delivery for both MediLink Community Health Services and Marchese Health Care.

“Leslie’s investment in training and knowledge gained to become a CGP will be very valuable to her work at Marchese Health Care,” said Pharmacist Manager Guy Cuerrier.
